- 📄 Download the Submission Form: If you choose not to purchase the EquiSample Kit, download the submission form here.
- 📮 Return the Sample: Place the collected sample in the prepaid express envelope and use the door pickup service. If you did not purchase the kit, you are responsible for shipping the sample to our lab via regular mail.
